C# desktop app to get the caller id from modem

The application will run on the background and it should just be able to detect an incoming call and send the sender id number with an http request to an external server API.

Simple User Interface.
The user should be able to enter:
Username:
Password:
Com Port:
and SAVE button.
and a TEST button.
The test button will just test if there is actually a modem connected in the COM port defined that returns OK after querried with AT commands.
There are plenty of information on the web. I think the preffered way to do the project would be with deriect AT commands to the serial com port of the modem, so that to first enable the caller id, then detect the ringing and finaly grab the caller id in order to send it with the http request.

The desktop app should be able to auto start every time Windows Open.
The application should work on both 32 and 64 windows systems.
The modem that will be used support the caller id recognition.
After the completion of the project I will need all the source files of the project.
