Below are the requirements of 4 reports. Please refer to the excel version if there is any conflicts.

1. Renewal report

The report contains following columns:

Policy number, customer’s first and last name, effective date, expiry date, phone number, address

Users can select a date range, so all policy belongs to different agents and expiry date is in the range will be listed. When click send, the report will be split automatically by agent, and sent to all agents’ email address that listed in user detail page (if the report of the agent is not empty).

3. Receivable report

The report contains following columns:

Application date, policy number, customer’s first and last name, effective date, expiry date, total premium.

The report will be filtered by date (application date) range selected, agent/school, insurance company, and product.

4. Claim report (claim item report)

The report contains following columns:

Policy number, customer’s first and last name, address, gender, customer’s DoB, agent’s name, deductible option, cheque No., pay to name, claim creation date, service date, diagnosis, coverage code, claim amount, amount paid, and all notes(decline, internal, and external).

A claim with multiple items will all be listed on the report.

The report will be filtered by date (claim item creation date) range selected, agent, insurance company, and product.
