Simple Facebook Connect Button (Base already exists)

Hello,
I need a simple "Facebook Connect" button to be set up, which will results on sending the name and email of the person, to an Aweber database.
I have a good code template that used to work, but the permission window at first is blocked. I will send you that code, as a basis to work. This code is working and sending datas to aweber as it should be. But one permission is not working, so this would be the part to change.
The face app already exists so I already have an app IP to provide.
