I need a simple contact form created with validation.
This is the validation method/code I'd like used: [url removed, login to view]
The form should be set out like this:

The 'dropdown' option should be a drop down menu. Whatever options are here, should dictate the labelling of the next field. So If the options for the dropdown menu are Option 1, Option 2, Option 3 and Option 4. Then if Option 4 is selected then the next field below should say 'Option 4 label' (as in the example) This text should be able to change on the fly without reloading the form/page.
Validations:
Your Name field should validate any word/numbers are allowed.
Your E-mail should validate an e-mail address has been entered with @ and a .com/net etc.
Dropdown should be validated by any of the options being selected.
The field below the dropdown (Option 1, 2,3,4) should validate if a proper URL has been entered.
The message box should validate by having a message longer than 30 characters.
Once the form has been submitted successfully it should load a page/message confirming the email has been sent.
