This document that needed to be converted to a online form, with validation, and then when the user press submit, it needs to be show a confirmation page then button to printed out what the user has filled up,
use jquery, css, and validation and up to the web 2.0 standards,
also The form and the printout should look like the word document you have. Please make sure, that the form looks ok in a 1027x768 resolution as well as in a 1280x1024 or 1440x900 solution. It should also be applicable for Firefox, IE 6 and 7 and Safari. I assume you are using CSS for the webform.
