Teacher Rating Script

We need a PHP script which allows students to rate their teachers. Students will be entered into a database with all teachers they have. Then, a code will be generated for students, allowing them to enter the rating site, where they will be offered a list of their teachers. Upon choosing one of the teachers, they will be forwarded to a site where they can rate certain aspects of the teacher as specified by the administrator. Also, the possible grades will be set by the administrator (e.g. A, B, C,... or 1, 2, 3,...). Each grade will be assigned a numerical value, in order to generate the mean value which will be shown on the teacher's "report card" (would not be a problem with 1, 2, 3... grades, but I guess it would be a little bit tricky to calculate with A, B, C...).
The script should have a nice look and feel for users (administrator and students) and be easy to install (simple install script asking for database information and administrator password). The admin area must be password protected and allow the administrator to add teachers and students, generate an invitation letter for students (preferably PDF) and the teachers' report cards. By the way, the students' names should not be stored in the database, but only used for the invitation letter.
