General Quiz generator base on PHP

This is a quiz engine driven by json settings (the attachment is a recommendation) . all input fields, radia/checkbox and display label are customizable. User point of view: 1) fill in some personal informations 2) answer 1 or more questions per page 3) go to next page and answer more questions, ( anwser of previous page is saved) 4) user may go to previous page and change the anwser. 4) submit the quiz. change of anwser is not permit after submit. Requirement: 1)questions can be select-one or multiple choice, (radio or check box) 2)choices of question are less than 10. field of personal information is customizable 3)custom label defined in Json can be accessed by PHP code. Output: 1) send the information to EMAIL or SQL or both, sql table are created based on the json setting. 2) for EMAIL, the format is CSV, for import to Excel. 3) for SQL, the table structure is created according to Json format on the fly. 4) simply INSERT the result to sql table, need not check for duplicate. 5) if the sql table exist conflict with the json setting, show error and do nothing. ( for example, field mentioned in json but doesn't exist in the table) programming style: 1) good seperation between UI and non-visual code. 2) sufficient remarks and note.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
