Quote Form and Quote Backend Manager

There are two parts of this project and a example of the quote form can be found here [url removed, login to view] - it's just a example. I will need captcha and a few extra fields.
**Part 1** - I need a quote form made that has email validation (not javascript popup but shows errors in red font above form), required forms, and recaptcha functions. The form will have the following fields: Name, Email, Phone, Company, Website, Photo / Mockup Upload (this is picture format only and needs to have a 200k max), an option to upload 2 more pictures (i.e. - Add another photo link) and when clicked it shows an extra field, and a Details field (message field with 400 character limit). The form will send a email to me with quote form information and a link to my Quote Admin Panel and add information to the quote manager.
**Part 2** - I will need a backend to manage these quotes. I want them nicely formatted with rollover effect (not plain, default blue text). Each quote in the backend will have to give me the option to mark it as "Contacted, Interested, or Not Interested". When I click on the quote I would like the see their contact information nicely layed out with any images they uploaded. I need the ability to delete quotes.
I always have projects and work and prefer to have a good coder who is smart and can find solutions and does the best work, always. If I find a coder that is reliable, honest, with decent prices I will continue to use you for very long time (with bigger projects).
