Convert MS Word Template into Adobe PDF Editable Form for Print

The aim of this project is to convert the existing MS Word Document into an Editable PDF Form. Ideally this form will be able to be submitted in the future for processing to a website backend, so keep this in mind.

The newly created PDF form does not need to follow the same layout as it is displayed in the MS Word Template, but should be

* Editable on desktop platform

* Editable on a mobile tablet (allow for adequate spacing, and minimal scrolling etc) - (Tablets used iPad Retina Display & Samsung Galaxy Note 10.1)

* User submit buttons for submission of the PDF via email

* The Calculation fields must also calculate. The formulas are embedded into the Word Document.

You will have a maximum of 4 working days to design, and build this PDF document from award date.

I must be provided a copy of the working form for testing prior to final payment.
