I have 1 form in PDF format and need you to make it into a PDF form that i can add data to...

The current pdf has data on their including photos and text, you need to take that data off including photos and text fields, create boxes where i can enter new data and photos for future orders..

with the photos maybe some sort of upload button ? with the text i will need to be able to type into the form directly..
