HTML and CSS Design Based on Graphic Design

I am writing a web application that requires an interface similar to an email client. This is a three column layout with two columns being fixed and the third column being dynamic. I have attached two documents:
1. App Layout - A general rendering of what the HTML/CSS should look create
2. App Layout Requirements - A document explaining the various elements and requirements
All I need is the HTML and all of the supporting CSS to make this layout work. I will be developing all of the server-side code to actually wire it up.
