logo design and a custom NCR receipt book made

My family farming business needs a NCR book to keep records for trucks that pick up grain on farm. And i want a professional timelss farm logo to put on them.
Designing the NCR book is pretty straight forward, I was looking at ordering through CMYK colour online unless you'd have a better suggestion.
2 parts of this job
(1) design a logo for our farm, Kentucky Pty Ltd
- making the name the focus
- incorporating an image of wheat (preferably) or a combine harvester
-suitable to use in the NCR book as well as on business letterheads and other stationary
(2) design the NRC book
-needs to be simple and easy to fill out, by truck drivers in the paddock so enough room for large writing.
-including business logo and contact details (details can be emailed)
- I have a good idea what the design needs to be and what fields are needed which can be emailed
- DL or A5 size
