Make changes to an existing picture - repost

Hi, I have an existing picture (logo) which a couple of things done to it.
1. Create a transparent background and provide in eps and jpeg format
2. Using the file provided to create two new pictures that will be used on stickers, basically I just need text added to bottom of logo. Text will be
"Emergency Kit" using the same font as the logo Futura Md BT.
Below is the jpeg version, but will email successful bidder the eps file.
