Write an iPad application

Simple native data collection app.... This project is for TWO 2 apps. A version with the app's name/fixed on the first and last frames and one without.

The Freelancer provides the code and wireframe design (to be approved/mutually produced)

PURPOSE: Survey app used at conventions as a kiosk to collect user data for contests or newsletter / client lists

SETTINGS FUNCTIONS:

- App user can add 3-4 custom logos (Frame #1: 600x600, Frames #2-#5: 200x200, Final Frame #3,#4,#5 or #6 (depending on size of survey): 600 x 600 and also 600 x 150 (fixed in version A, custom logo in version B - see attached PDF files).

- App user can select 6, 12, 18, or 24 question surveys (which makes the entire survey either 3, 4, 5, or 6 frames long.

- App answers could be: YES/NO, DROP DOWN LIST, or CUSTOM ANSWER (like Google Forms)

- App user can create custom questions (either 6, 12, 18, 24 depending on how long of a survey they select).

- App user can add custom text on the first and last frame.

- App user can make a new survey (add survey name) or choose an existing saved survey (allow up to 20 different surveys to be stored/deleted).

- App user can export saved data to Excel (xls, xlsx-compatible), CSV, or PDF format, via email and other exportable method (Dropbox, Google Drive, etc.)

- 4-number protected Lock-Out feature (once in running mode) to prevent user from accessing other apps, confirm 4 digits, (activate in Settings Panel, and pop-up 4-number Lock-Out box if user presses the ipad Home button or ipad Power button (unlimited number of attempts).

APP FUNCTIONS:

- Entire app is only 3, 4, 5, or 6 Frames

- Frame #1: Custom 600 x 600 logo and up to 2 lines of custom text at 16-22 dpi - "START" Button - Fixed line of text and small app logo on the "A" version and no app information on the "B" version (see User Screen 1 PDF file, actual wireframe to be designed by Freelancer)

- Frame #2, (optional frames #3, #4, and #5): Custom 200 x 200 logo and user provided answers to populated questions (yes/no, drop down, or custom answers.

- Final Frame #3 (or #4, #5, or #6 depending if user has 1, 2 or 3 pages of questions): Custom 600 x 600 logo and fixed logo at 600 x 150 (in version A), custom logo 600 x 150 (in version B). Custom Text Box (one or two lines at 16-22 dpi) in both versions.

[RECORDS DATA / ANSWER after user hits "Submit"]

After 15 seconds survey resets to Frame #1 (VERSION A) - After 5 seconds survey resets to Frame #1 (VERSION B)

Additional: User should be able to swipe to NEXT page or swipe BACK to return to a page and change their answers or CANCEL their survey.

Freelancer agrees that they do not retain ownership of the app. It will be submitted by the Project Owner who will retain all design and ownership rights.
