40371 PHP database setup

I need a PHP database set up for [see text file for URL] The variable set up is as follows: recordid int(11) auto_increment city varchar(100) state char(2) category enum('retirement community', 'retirement home', 'nursing home', 'assisted living', 'senior resources', 'other') company varchar(255) desc text url varchar(255) address varchar(100) zip varchar(12) phone varchar(20) add_date date 2006-01-01 update_date date 2006-01-01 active enum('y', 'n') n [default] display_order tinyint(4) 10 premium enum('y', 'n') n [default] image varchar(60) contact_info text [contact name email phone] Specifications: • The data will be displayed by state and by major category with the following pages – [url removed, login to view] (for the state pages, [url removed, login to view], [url removed, login to view], [url removed, login to view], [url removed, login to view], and [url removed, login to view] • I will provide a list of state abreviations in the attached file. • The top menu, footer, left and right side menus need to be include pages so when changes are made, they will apply to all pages. • A page called [url removed, login to view] will be used to enter data into the database [see text file for URL] • When a new record is added, it will not automatically be included. I will go into PhpMyAdmin to make the record active. I have not finished the design or finished adding links, but I think there is enough information for you to see if you want the job.
