I need an existing Wordpress theme modified by someone who knows CSS and Wordpress well. 1. Movement of the navigation menu to top as shown in attached sketch. 2. Add an empty panel for advertising on the right hand side as shows in attached sketch 3. Adding a form with 12-15 fields and a captcha that generates an email. (Fields listed below in order) 4. Change the CSS background to something else in particular - I'll send you a sample image that should be used repeatedly for the background. 5. Round the corners of everything as shown in sketch. Form fields: 1. Your name 2. Title 3. Email address 4. Company name 5. Website 6. City 7. Phone number 8. Description of Company/Product (larger form field) 9. Primary Market/Competitors (larger form field) 10. Funding status (drop down) Unfunded/Self-funded Angel Capital Venture Capital 11. ## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
