Hi everyone!

I need a restaurant table reservation form for my website.

The form has to be and work EXACTLY in this way:

The form has to be named "[url removed, login to view]" and after submiting, it has to redirect the client to another website exple "[url removed, login to view]" like the example.

I have to be able to change the destiny of the email that get all the information from the form.

I need it ASAP!
