Simple PHP email signup with XLS export

I need a very simple email signup form that takes all submitted email address and first and last names, and puts them into a mysql DB, as well as updates to an excel spreadsheet, listing one submittion (Email, First name, Last name) per row.
The code should be written in PHP.

So you want a form and the field should be post in a mysql database? and than with XLS Eport to you excel?

Updating mysql is the same old php. For XLS, I am proposing to use OpenXML. It will avoid the need to have Office installed on the server to update XLS.
