PHP submission form

Hi there,

I need a small php script that will work on a facebook page tab.

The user will face a question with 3 choices - 3 radio boxes.

I will like the script that will do the following:

- 3 choices ->3 radio buttons; -> if answer(radio box)1 or 3 selected ->the user will be sent to [url removed, login to view];

-> if the radio box 2 is selected ->the user will be sent to [url removed, login to view];

NOTE: the user will be send to those html pages if he fills in his name and e-mail address into another 2 text areas;

- Besides the question, the user has to fill in two other areas with his name and e-mail [url removed, login to view] the user doesn't fill these boxes with his name and email addresss he will be sent to [url removed, login to view] or if he does provide his name and address but doesn't select a choice, he will be sent to [url removed, login to view]

-the information filled in by the user must be sent to an e-mail address and the e-mail must show which answer the user has selected ( wrong or correct)
