Create an windows application calendar for a Doctor

Create a calendar application that shows the week Sun to Sat info with the design similar as the image attached below. This application must be available on and offline, so it can be a offline windows app that send the calendar in each 15minutes to an online website, that displays the calendar in responsive mode (it do not need to be editable online, only in offiline is ok) so the calendar can be viewed in an iPhone. Do not display the time hour-by-hour, only display the scheduled time in the scheduled tasks box and order them in ascending order. I need the source code to make future changes if it is a compiled app. The scheduled box tool must have phone, cellphone and email fields, but this info will be showed only when the item is clicked. When in calendar view it will show only the name of the patient, the time scheduled (from and to times) and the description of the procedure.
