Custom Coded Form Email Notifications Not Working

There are a few forms on the website that are custom coded.

These forms when submitted by customer go to specific emails and it was working 24hours ago but now is not, I have not had any issues in the past year so I have no idea what is going on. When these forms are submitted an email confirmation is sent to the customer and then what they input on the form is emailed to us, all of this is set up but not working anymore, it was working 24hours ago.
