Simple API script for Semrush

I need a very simple Semrush API script for personal use. It must be in PHP and it will run on my linux centos server. The front end can be very simple as it is only me that will be using it. It should work like this:
The script will allow input through an online form accessible via a web browser (php script hosted on my server). The input will be in the form of keywords, line separated, like this:
plumber baltimore
accident lawyer south carolina
custom shoes
Once I click Submit, the script will retieve from the Semrush API the PAID RESULTS for keywords (phrase_adwords), and present the output as a downloadable csv file, with two columns: input keyword, and domain (retrieved from semrush). Like this:
plumber baltimore,[login to view URL]
plumber baltimore,[login to view URL]
accident lawyer south carolina,[login to view URL]
custom shoes,[login to view URL]
The api is well-documented with examples:

the relevant section is PAID RESULTS for keywords (phrase_adwords)
The script should be able to handle input of at least 1,000 keywords.
