vtiger 540 to 650

we have an existing live vtiger CRM hosted internally.
We need to have a duplicate created in 540, then migrate to 600 then up to latest version (650)
All existing data for all modules in existing CRM need to be imported to new CRM.
In existing CRM we also use the "Export to PDF" tool which created a PDF from a Quote. The template of the PDF quote was custom designed for our company and we need it to be retained exactly as is.
Page numbering should also continue where we left off.
Most modules have had their fields modified to our needs. All structure of fields needs to be retained. We also have an existing module (TimeTracking) which has already been modified to work on 650. We would need this imported into the new version, whilst retaining the same existing data.
Data in Products and services also need to be imported, as well as reporting.
All existing picklists should be retained in upgrade.
Access to existing site will be given, as well as FTP access.
