Have website with wordpress and theme installed.
Need the following:
1) wordpress was upgraded manually from old version, and media doesn't display on site or in media library.. Need fixed.

2) Need homepage contact form to look like demo and have same fields

3) Need homepage configured. No need to create content.
4) Need 3-5 Simple small color coordinated icons made for homepage.
