I need to prepare a quote for a website project. I have a list of minimum requirements for the site, as well as an additional list of features that need to be quoted as well.
Minimum features and requirements:
This site needs to be coded in HTML 5 and CSS3. It needs a responsive design with 3 sets of styles (phone, tablet and desktop) It will consist of 5 pages including a contact page with a submit form. I am prepared to host this on either a Linux or Windows server, so ASP.NET or PHP are acceptable if used for the contact form. I'm not comfortable with Joomla or other content management systems. This site will be for a medical care provider and pdf links will need to be available. A logo has been provided. Some graphic work may be required to incorporate it into the site depending on the background colors. Other design experience will be required to make sure the site has a nice modern look, and preferably light-weight as well.
Additional features:
Blog section - this would of course require authentication, mysql or sql and a server side language. I'm open to suggestions if there are pre-built utilities available rather than designing this from scratch.
Information sheets for patients to fill out - This would consist of quite a few questions, text boxes, radio buttons, validation, etc. This would be an extra option instead of downloading the forms in pdf format.
