VBA Word to Excel Macro

I need VBA code that will search through an entire Word document (all text) and find all headers and references and put them into an Excel sheet. This needs to be an automatic process - a Macro.
Example:

This is the body of the article (See my other article of July 25, 2008: Starting a Blog.). Here is another sentence (See my post on June 15th, 2008: My First Article).

The above would appear like this in an Excel file:
8/27/2008 | This is the Title (A Sample) | Starting a Blog | 7/25/2008
8/27/2008 | This is the Title (A Sample) | My First Article | 6/15/2008

Not all articles will have a reference. All references should begin with "(See" (minus the quotes) and end with ")" (minus the quotes). The title line should always be in "Heading 3" style.
I've attached a sample document and a sample Excel output file. If you think you understand the project please manually complete the one missing line in the Excel file (there is one reference missing) and include it with your bid. This should take less than one minute to accomplish and ensures you understand the project completely.
NOTE: I could do this myself, but am behind schedule. If you can get it at least 75% there, I can finish the code.
