Simple Web Design - Reverse Mortgage

We need a very simple web design for a new mortgage company. I have the ability to code the HTML, and the backend in PHP, however, I have no design skills at all. If you can create the design in photoshop I'd be able to write the HTML, but I would prefer not to waste my time. The name of the company is Simple Funding Group. We are a mortgage broker that focuses on reverse loans, although we also do traditional forward loans. The website must be able to clearly demonstrate our range of services. We have a very basic logo already, with an American flag theme. Most of our customers are age 62 and over, so the design has to be accessible and attractive to seniors.

There should be a form with Name, Phone Number, and Loan Type fields on the homepage embedded into a background image with some sort of submit button. That form will continue in another more detailed form page. Other than that, the design is open for any idea you come up with. There is no layout preference. When you create menus, you can use standard examples for pages, and we will create pages and fill in content.
If you have any changes you would like to make to the logo, let us know. We are open to ideas.
