Contest Brief

The spectrum of the contest has been widened to any illustration that depicts gout. It does not have to be a foot only, but can by anything that depicts gout and how painful it is. But still no photos and deliverable includes layered PSD file.
Below is the original description of the contest:
I need a photo-realistic illustration of a foot (looking from the top).
It needs to show that the person has gout (in the typical spot below the big toe) with potentially some inflammation/swelling and flames to illustrate that it is very painful (focused on the area of the gout only)
It needs to be a slick foot (slick realism) with slick flames (Translucent flames - kind of like in the movie Ghost Rider)
Please have a look at the attached sample.
Optionally there could be some crystals or spear head or pins in order to indicate the painful acid build up (should be a separate layer).
The toes need to look nicer and the colour of the foot needs to be tanned.
No hairs should be present. The foot needs to look aesthetic (besides the gout spot).
Just to emphasize again: Not photoshopped photograph, please. 3D render might look nice for it being photo-realistic, but that is up to you.
I will require the picture as a high resolution PNG (no background) as well as the source files with layers (preferably PSD).
The background might end up being like in the sample, but this can change and feel free to suggest something that works best with your illustration.
And naturally: it has to be your work without any copyrights attached to it.
The contest might get awarded early, if there is a really good submission.
Some fine-tuning might be required after you have been selected as the winner.

    I just adjusted the contest to reflect that it needs to be an illustration of a slick foot (slick realism) with slick flames (translucent - similar to the Ghost Rider movie ones). Optionally there can be an additional layer with crystals or spear heads or short pins or something else to indicate the painful acid build up. Deliverable includes the layered PSD file.

    This is not a photo. This is a 3d model. The flames are 3d also, I created a ring of flamea to indicate the heat. I will amend the flames to be a bit more subtle. Thanks for the feedback.
