I need help to create a forms for our WordPress website please see the files attached. I don't seem to find a correct plugin for it. when a user login they can fill this form send it to an email as excel spreadsheet. under activity it must be a dropdown menu once they select one activity points tab is filled with a predefined number and total must show up a the end of the ligne. Fill free to use a plugin and customize it. who ever get awarded this project I will send him the information to login on my demo website. Thank you.
