440542 easy project - need banner rotator

ok this is an easy project for experieced PHP programmer, i need a banner timed interval rotator script that also shows when certain categories or cities are selected... script must be seperate from the page it will show on so i will use an include line to put on the page were banners will show on... if possible and doesnt cost more i would like it to come from an mysql database...need input form also to upload banner and input link info into
