Workers from KOLKATA or WEST BENGAL preferred,Simple Form-Filling Job

I need a data entry worker who can work for me 6 hours per day (may be less if anybody can work too fast, but normally it'll be 6 hours per day,10 days per month). The work is very simple. You have to just input various entries from the PDF file (which will be given to you). You have to fill out forms using a special software which will be provided to you also. All you have maintain some special rules (all are very easy though)while making a form. But you have no option to copy-paste as the software doesn't allow copy-paste. Anyways you'll be given all the informations and some rules before starting the project. You have to work only 10 days per month. So it'll be also easy for anyone. You have to fill entries in forms (varying the number of forms per month 1500-3000). In general filling different field of a form will not take more than 3 minutes. The rate will be 10 cents per form. So minimum income will be $150 and maximum will be $300. Payment method will be Paypal, GAF or Local Bank Transfer. No advance or escrow.I'll prefer full time data entry worker for this project. Payment Schemes: >=98% accuracy----------> Full Payment >=96% to <98% accuracy--> 60%payment of that month's whole work <96%--------------------> Zero Payment I'll give priority to data entry workers from KOLKATA or West Bengal.
