155643 Create PDF Document

We need a work order document created. It must be in PDF form and you also must provide the form in Word or some other format that we can easily create the form. Attached is a sample. It must contain our logo (we will provide). We must be able to add information to the form online and still contain enough room for the technicians to add their notes offline. The document should be secured so that no changes can be made to it other than the input fields. Once we accept your bid we will provide you with more details. The sample work order is the basic layout we are looking for. This should be a very simple project with vey little time involved.
