Desktop Software for Medical Clinic

I need to develop a general purpose software for administering medical clinics. The software must have the flexibility to manage multiple clinics with multiple doctors of different specialities. The software should be developed in WPF / C# and should be avoided the use of third part components , except for Telerik and DevExpress. The database must be multiuser of easy installation , configuration and administration. The instalation of the software and the database should be done with the use of Click-Once without requiring additional procedures. The code should be well structured , making the use of design patterns where necessary , be well documented and self explanatory because it will be maintained by a mid level C# [login to view URL] software should have localization for Portuguese and English.
