your database was not properly created, i can fix it for you to store separate usernames. if giving the opportunity.

which means he write mistakenly or intentionally sql query to update all user's username . for ex if below query execute it will update username for all user's "username_for_all" . update usertablename set username='username_for_all' ,if u have a backup od DATABASE means we can bring site live otherwise it ill create huge issue
