French web appointment

in ENGLISH (and bellow ++++ in FRENCH/prefered) ENGLISH I come with a question that relates with access/SQL and web. I have set at [login to view URL] a graphic which shows what I am looking up to - my client should be able to come on such page through my web-site and make an appointment. Filling-up ID and pass and possibly a tel number - then choosing available time (with option cabinet or telephone). Once done - the page comes again with a grey scheduled time for general public ; except for me who can open the data base with full/clear info about patients name and data. This program should include 2) a ID+pass entry and cooky step. and 3) possibility for the client to check out his own appointment s/he has allready taken (as in red color for example) with possibility to cancel it. ++++++++++++++++++++ FRANCAIS cherche aide programmation pour installer un agenda prise-de-rdv pro/clientèle (cab medical) sur mon site web (déj? acquis).
