Joomla Component - Incident Report Log and Progress

We are looking for someone to create a joomla component that will be able to manage all our company's accident and incident reports. We want our staff to be able to report an accident or incident online via the company intranet. With a Management & Supervisors back end for them to privately view all entrys. ## Deliverables The component should have the function to provide a brake down of all accident types and able to create to types of reports i.e. Near Miss Optional extra for it to create a graph/chart from the collected data. Once a report is submitted, it should auto send emails to set mailing list depending on the type of accident or incident that is reported to ensure the correct managment are made aware of the event.
