Add Video to PHP/MYSQL Site Profiles

Site: [login to view URL] Brief: I need users to be able to add/upload 1 video to their profile via the singup page: [login to view URL] The video should show up in the video area of the profile page. i.e.: [login to view URL] If the user didn't specify a video it should still show the player.
