Flash - pass a php vars

Hi, I have a webcam recorder online(look the screenshot in the attachment) in flash that let user record themself online on my php website.
In my flash apps I have the field 'Titre' that means Title in english because i speak french ;) and I have the field 'Nom d'utilisateur' that means username.
Actually on my php website I have a session vars $_SESSION[USERNAME] so I get that vars and fill automatically the field 'Nom d'utilisateur'(or username in english ;)) and this field cannot be change.
What I need :
1-I need to pass the value from the field 'Titre' in a php vars
2-I need to pass the value from the filename that is recorded in my flash media server when the user push the submit button.(note submit is 'Soumettre' in french) in a php vars
Once the user click on 'Soumettre' button you bring the user on another page that say 'Your video has been sent successfully' and from there I must be able to make an echo of the Titre vars and filename vars.

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

(No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
