PHP app or Wordpress plugin for frontend quiz/test creation

Thanks to all those who bid for this, but this project was poorly described and hence posting this again with all details to be fair to those bidding*** *** THIS IS NOT A WEBSITE DESIGN PROJECT. I'm looking for PHP experts with good jQuery/Javascript skills. If you can build me a Wordpress plugin, consider this job yours (willing to pay more for a Wordpress plugin). So please don't waste your bids if you don't have the skills and experience to back it up.*** At the most basic level, a frontend form creation PHP app is required with the following form controls: - Browse and upload an image. One text box to add instructions. - Create a multiple choice select box. Users should be able to add as many of these as required (jQuery/Javascript, AJAX skills req.) Users use these form controls to create a "test" which they can preview and make changes to if required. The "test" can now be made available to "testers" via a web page on my website. Data from the test should be collected and presented in a report to that user who created the test (% on target answers from all tests). Apart from this, a bit of complex scripting is required to tag the uploaded images (open source javascript plugins availabe). My preference would be to have all this functionality in one (or two) neat Wordpress plugin with a clean user-friendly backend for editing reports, but I'll settle for a good PHP script which integrates into my Wordpress site. I'll be selling a service, so Paypal integration necessary. Membership management will be required (can use available plugins like Wishlist Member). In 6 months I plan on making improvements on a similar scale, so looking to hire and keep hiring the winning bidder.
