iPhone - small DB with few screens

Note - details and screen flows attached iphone app - approx 4 screens with small database and basic sync from mysql via php pages that I've prepared and/or will prepare depending on your requirement to make this work (hence some basic SQL and PHP knowledge is required) - in other words, you make the iphone part and I'll do the PHP part but we'll need to work together This is a first cut so more changes may be requested (additional work at additional cost) afterward. I'm open to your suggestions for design (ie use of spinners VS tableviews) but really need good, bug free functionality that is easy, fast and intuitive for the end user Note that there is an OPTIONAL requirement to include barcode scanning (interpret text from a barcode via the camera) - this is a nice to have and i really won't want it if it adds too much cost/complexity to the job - please note in your bid if this is or is not included (or note that barcode functionality would be an extra $XXX ) I will want full ownership of any code / project files created to make any changes in future however would most likely have the successful bidder make any future changes if I'm happy with their work any questions please contact me
