I am not sure on the best method/approach for the below, but this is where your expertise will come in. Hopefully it makes sense.
Here goes...
I need the following designed. All pages, must be SEO friendly and must allow me to go in after design and add logo's, menu's, etc.
Page 1 = Login Page with username/password fields, which will direct users to Page 3 mentioned below upon successful login. Also contains a link to Page 2.
Page 2 = Registration page for new users to signup.
Page 3 = A comprehensive form (approx 22 fields) which will include textboxes, textareas, dropdown, checklist, upload/browse button for attaching images, preview/cancel buttons.
Page 4 = A final preview of Page 3 form content results in HTML or PDF format, with submit and back buttons. Upon hitting the 'submit' button an email is sent to a specified address with the contents of the form in HTML or PDF format (if possible?).
Page 5 = Thank you page.
As mentioned, I don't even know if the above can be done, but this is where your expertise will come in.
Please advice if the above can be done, and by using which technologies. (eg: PHP, JavaScript, etc).
- Please provide samples of similar work if possible.
- Database = MySQL
- Looking for a 3-5 day turn around after project accepted.
- After final product is completed you must be available (IM, email) for some tech support (if required) for implementation onto server.
