I need a system to generate standard reports. The system must be web-based, to allow the questionnaires be accessed at www, and the reports generated by the web-based system should be delivered either in word or in pdf. The system must be able to: a) Handle standard questionnaires with various information about the respondent. It may be department, function or other. b) The system must be able to generate a list of respondents who have not responded. c) The system must be able to automatically generate a report based on the answers from one or more respondents. The report shall be adjusted according to the response. This means that section of text is drawn into the report depending on what the respondent / respondents have answered. d) The system must be able to generate the above reports based on a set of answers from one respondent or based on answers from multiple respondents, it should be possible to filter the respondents based on the information which is referred to in section a. e) Once the report is coded, it will be a standard report to be used several times.

Some questions: - Will admin will put all the questioners from backend which will be accessible to the user on front end for them to fill in the answers? Please advise. - Will the user have to register and login to the website to fill in the answers. - We assume all the reports generation will be in backend i.e. it will be done by the admin?
