Coupon generator

I need a software for my website that will Capture contact information (Name, email, telephone) Send an email to the person who filled out the form giving them a unique code. Example "Here is your coupon code: hgfjs219. Copy the code and go to [login to view URL] to redeem your product" I must be able to upload my coupon codes that will be generated and the codes can only be used once I will need a database to see which coupon code was sent to each person. The contact information that is captured must also be linked to my Aweber account. The email message above must also give the person The option to receive email from us. If they said yes then it will link to Aweber account. If no then it will stay in the database of the capture form.
