Looking for a beautiful dynamic PHP form to MySQL DATABASE that will output into PDF, EXCEL and immediately email out both submitter and owner. The owner should be able to edit the form from back end and changes will be mailed to submitter. Every PM should come with an example of a form that you have done, attached is actual form skeleton/drawing. I'm not expecting any bid above $50 as i have already done the form [login to view URL] its pretty simple only that i dont have time to make it beautiful than some of your good fellow can ;-)
