Design WordPress plugin that adds a widget to comment form

This project is for a WordPress plugin development project. The project requires the winner to build a plugin that does the following:
1) adds a Javascript to comment forms, which goes to my website, retrieves a human challenge and inserts it into the form
2) when the user submits a comment, the plugin must take their answer, send it to my website and validate the answer
3) if the answer is correct, the plugin should post the comment, otherwise, it should display an error (there are 5 possible errors) and return the user to the comment form
4) the plugin should have a settings page to allow the user to enter a public and a private key
