FastCGI sample application works on IIS 7

We have an existing code that we want to integrate into a FastCGI application that works on IIS 7. We want three functions each will be called as follows: AppInit() => Will be called when the module is being started by IIS7. RequestArrive() => Will be called when a request is made to this module. In this function we need sample code that will read GET parameters and write an output string as a reply. AppClose() => Will be called when IIS7 closes the module. We also need a documentation on how to install and setup this sample module on IIS 7. We want to be able to do requests like http://localhost/somename?a=2&b=3 and be able to read these GET parameters in RequestArrive(). This job is only for a sample application. No further coding is needed.
