Yes I been using Scrum methodology as a process for our Project since 3 years. I am Scrum Master/Project Lead working under Agile Process Development and used tools like Rally and Pivotal in the recent past.
For any Project to start with we follow the mentioned points as part of Scrum.
a: Scrum Team needs to be identified incuding Dev and QA , each resource with percentage of involvement he/she would burn during the project needs to be identified. A Scrum master need to be identified who can work on overall Project Progress , in terms of Story Creation, Task Creation, Burn down progress of Stories and also provide inputs/guidance where Story or Task need to be sub classified.
b: Story with proper acceptance criteria needs to be created in Rally. This would be the single point of source for QA in Test Case Preparation and DEV team for task creation.
b: Story Estimation is done through a Planning Poker Session.
c: Task needs to be created by individual Developer who is part of the respective Story.
d: All Above points needs to be completed on Day 01 of any Sprint.( Usually a Sprint consist of 10 working days)
e: Team Task is monitored using Iteration Burn Down feature in rally and discussed in Daily Scrum call for maximum 30 mins.
f: Build Release plan needs to be shared with the QA team and Iterative builds needs to be provided.
