Small HTML and jQuery Project

This is a very simple project but has to be done as soon as possible: We need someone to change a existing html5 website and add some elements to a form. This is the current page: [login to view URL] And this is how it should look after you have done your work: [login to view URL] So the task is only to add the elements which are on the screenshot an which are currently missing on the current page. Also there should be some javascript calculations involved. E.g. if you select another number than "1" in the dropdown menu the new price should calculate automatically. The radio buttons don't need to look 100% like on the screenshot. Although iCheck jQuery library could be used. If we are happy with your work we might have further tasks for you.
