Email capture landing page

I need a freelancer to put create two variations of a simple email landing page. The freelancer will need to professionalize the graphic design of each in the powerpoint attached and then create HTML files, including the web sign up form. We use iContact for email marketing, so we will use one of their forms, which will send the emails to our iContact account, but we will need to modify the HTML to fit the design. If necessary, we may need to do several different drafts of the design. The designs should stay clean and crisp. There is no need to add anything other than perhaps a subtle background pattern (if you think it makes sense), perhaps a couple of symbols, and select colors and font that make sense and optimize conversion. Note that the purpose of these landing pages to maximize the conversion of people that enter their email address.
