Add 2 Screens to an already built Android APP

We already have an App Built. We need a new menu item added which will have 2 screens. One of the screens will be exactly the same as another screen in the app and you will have to design one screen with a form. The form will also need integration with a QR Code scanner which is already being used in the app currently. Also, the form content will need to be posted using a Rest API and you can find similar examples in the app. There are other forms posting. Ill give more detail personally and this shouldn't take you more than a couple of hours. Very Simple Job.
